		<p>We are celebrating the 45th anniversary of consistent publication of the Colombian Journal of Anesthesiology, after overcoming several challenges with regard to its inclusion and positioning, striving to meet quality and technological requirements of other world class journals.</p>
		<p>Notwithstanding its accomplishments, the journal faces today a number of new challenges in the area of scientific research; that is, financial pressure derived from being an open access journal, its indexation in the most renowned and extensively used databases, the impact factor positioning the journal at a world level, and the arrival of new stakeholders in the scientific arena-the so-called predatory journals, that publish low-quality articles without an adequate review process and paid for by the authors.<xref ref-type="bibr" rid="B1"><sup>1</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B2"><sup>2</sup></xref>
		</p>
		<p>Furthermore, from the professional point of view, anesthesiology, and surgery have become increasingly safe through the adoption of the aviation industry practices, by performing in front of the patient,<xref ref-type="bibr" rid="B3"><sup>3</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B4"><sup>4</sup></xref> using the best and state-of-the-art monitoring, as well as through the availability of technological breakthroughs that enable an improved performance of ever more complex surgical and anesthetic procedures. However, in addition to the challenges in research and scientific communication, there is the impact of the lengthy and even stressing work days on the quality of life of healthcare professionals and their families, as was recently published in an article in this journal.<xref ref-type="bibr" rid="B5"><sup>5</sup></xref>
		</p>
		<p>High-quality scientific communication demands robust and strong research processes that withstand peer review; hence, healthcare professionals exchange remunerated work hours for non-remunerated long research hours, so that they are able to position their research in the scientific arena.</p>
		<p>Over these 45 years, we used the opportunity to extend an invitation to publish research papers in the Colombian Journal of Anesthesiology on the quality of life of healthcare professionals, that may make relevant contributions to change and improve this aspect over the next years, in addition to the classical and highly appreciated scientific and technological research articles on a range of topics including anesthesia, pain, critical care, palliative care, and perioperative care, inter alia.</p>
		<p>Perioperative care, as mentioned before, is part of an emerging care language beyond the surgical-hospital environment,<xref ref-type="bibr" rid="B6"><sup>6</sup></xref> and is going to demand from healthcare professionals and specialists, changes in the way things are done, changes that may further increase the complexity of our practice or, if we get ahead of those changes, they will lead to improved quality of life of these practitioners that has not been thoroughly considered in scientific research. This is a challenge we must take on as researchers. Issues such as &quot;burnout,&quot;<xref ref-type="bibr" rid="B7"><sup>7</sup></xref><sup>-</sup><xref ref-type="bibr" rid="B10"><sup>10</sup></xref> depression, suicide, and the use psychotropic substances,<xref ref-type="bibr" rid="B11"><sup>11</sup></xref><sup>-</sup><xref ref-type="bibr" rid="B13"><sup>13</sup></xref> and probably a number of proliferative diseases,<xref ref-type="bibr" rid="B14"><sup>14</sup></xref> have been studied as factors associated to the profession; however, little has been said about any interventions and actions to prevent or manage such issues. Hence, there is a need to close this gap in our knowledge through research, for the benefit of the healthcare professionals that practice in surgical environments.</p>
		<p>The Colombian Journal of Anesthesiology is now positioned in Latin America, as a top quality journal. Since the Journal was admitted to world renowned databases, it has gained visibility among researchers worldwide and we hope these researchers will continue to consider the journal for their publications, considering that it is an Open Access journal, bilingual (English and Spanish-and the translation costs are covered by the Journal itself); furthermore, the support given by Wolters Kluwer, ensures sound processes and high-quality articles published, subject to a double-blind peer review process and under the surveillance of the &quot;International Committee for Medical Journal Editors&quot;<xref ref-type="bibr" rid="B15"><sup>15</sup></xref> and the Committee on Publication Ethics.<xref ref-type="bibr" rid="B16"><sup>16</sup></xref>
		</p>
		<p>We are well aware that the peer review process is increasingly difficult, particularly for open access journals that depend on the voluntary-non-remunerated-contributions of other researchers. This difficulty of the peer review process creates a fertile ground for predator journals and makes them attractive for researchers who are not aware of their mode of operation. Hence, I want to express my deepest gratitude to our peer reviewers that ensure the quality of our processes and articles; to the members of the Scientific Committee, and the members of the Editorial Committee, for their significant contributions to pave the way for the future of the journal; to the researchers that trust the journal to submit their papers, to the million readers that visit us each year, and particularly to the Colombian Society of Anesthesiology and Resuscitation (S.C.A.R.E.) that has adopted the journal as a flagship project of the organization, and therefore has been financing its activities for the last 45 years.</p>

			<p>La Revista Colombiana de Anestesiología cumple 45 años de publicación periódica, luego de haber superado varios retos relacionados con su inserción y posicionamiento en los requerimientos de calidad y tecnológicos de otras revistas de alta talla mundial.</p>
			<p>A pesar de lo anterior, hoy en día, la revista se enfrenta a retos nuevos relacionados con la investigación científica como son: las presiones financieras para su sostenimiento por ser de acceso abierto (en inglés: Open Access); la indización en las bases de datos más prestigiosas y de alto uso; el factor de impacto que posiciona comparativamente a la revista a nivel mundial; y la llegada de nuevos actores al mundo científico, las llamadas hoy en día &quot;revistas depredadoras&quot; (por su nombre en inglés: &quot;predatory journals&quot;), que publican artículos de baja calidad, sin el adecuado proceso de evaluación, y con cobro a los autores.<xref ref-type="bibr" rid="B1"><sup>1</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B2"><sup>2</sup></xref>
			</p>
			<p>Por otro lado, desde el punto de vista profesional, la anestesiología y la cirugía se han vuelto cada vez más seguras, al incorporar elementos de la seguridad en la aviación, en la actuación en frente del paciente,<xref ref-type="bibr" rid="B3"><sup>3</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B4"><sup>4</sup></xref> la mejor y más avanzada monitoria, así como también, la disponibilidad de avances tecnológicos que facilitan el mejor desempeño de procedimientos quirúrgicos y anestésicos, cada vez más complejos. Sin embargo, los retos que se soportan desde la investigación y la comunicación científica, se acompañan de los impactos que desde las extensas y aun estresantes jornadas laborales se dan en la calidad de vida de los profesionales de la salud y de sus familias, cómo recientemente se publicó en un artículo de esta revista.<xref ref-type="bibr" rid="B5"><sup>5</sup></xref>
			</p>
			<p>La comunicación científica de alta calidad exige procesos fuertes y sólidos de investigación que resista las evaluaciones de pares, y para ello los profesionales de la salud intercambian horas de trabajo remunerado por, muchas veces, horas de investigación no remuneradas, con tal de lograr posicionar en el ambiente científico sus investigaciones.</p>
			<p>En estos 45 años, aprovechamos para hacer un llamado a publicar en la Revista Colombiana de Anestesiología, artículos de investigación alrededor de la calidad de vida y laboral de los profesionales de la salud, que puedan aportar elementos sustanciales de cambio y mejoramiento en este aspecto, para los próximos años, además de los clásicos y muy apreciados artículos de investigación científica y tecnológica alrededor de la anestesia, dolor, cuidado crítico, cuidado paliativo y cuidado perioperatorio, entre otros.</p>
			<p>El cuidado perioperatorio, como se mencionaba atrás, hace parte de un lenguaje naciente de cuidado más allá del ambiente quirúrgico hospitalario,<xref ref-type="bibr" rid="B6"><sup>6</sup></xref> y va a exigir de las profesiones y ocupaciones de la salud, cambios en la forma de hacer las cosas, cambios que pueden hacer que los aspectos laborales se tornen más complejos o, si nos anticipamos, faciliten el mejoramiento de estos aspectos de la calidad de vida de los profesionales, poco estudiados por la investigación científica misma. Este es un reto que debemos asumir desde la investigación. Aspectos como el &quot;burnout&quot;,<xref ref-type="bibr" rid="B7"><sup>7</sup></xref><sup>-</sup><xref ref-type="bibr" rid="B10"><sup>10</sup></xref> la depresión, el suicidio y el consumo de sustancias psicoactivas,<xref ref-type="bibr" rid="B11"><sup>11</sup></xref><sup>-</sup><xref ref-type="bibr" rid="B13"><sup>13</sup></xref> y posiblemente algunas enfermedades proliferativas,<xref ref-type="bibr" rid="B14"><sup>14</sup></xref> han sido objeto de estudio como factores asociados a la profesión, pero poco se menciona sobre las intervenciones y actuaciones para prevenirlas o manejarlas. Lo anterior exige llenar este vacío de conocimiento, desde la investigación, por el bien de las profesiones de la salud que laboran en los ambientes quirúrgicos.</p>
			<p>La Revista Colombiana de Anestesiología se ha posicionado en Iberoamérica como una revista de alta calidad, y desde su ingreso a las bases de datos de consulta mundial, ha empezado a ser visible para los investigadores alrededor del mundo que esperamos sigan considerando la revista para sus publicaciones, dada su condición de Acceso Abierto, ser bilingüe (inglés y español, con costos de traducción asumidos desde la Revista misma), y que con el soporte de Wolters Kluwer, garantizan procesos y publicación de artículos de alta calidad, luego de un proceso doble ciego de revisión de pares, y bajo el cuidado del &quot;International Committee for Medical Journal Editors&quot;<xref ref-type="bibr" rid="B15"><sup>15</sup></xref> y del Committee on Publication Ethics.<xref ref-type="bibr" rid="B16"><sup>16</sup></xref>
			</p>
			<p>Sabemos que el proceso de revisión de pares es cada vez más difícil, sobre todo para las revistas de acceso abierto, que lo hacen con las colaboraciones voluntarias de otros investigadores, sin algún tipo de remuneración. Esta dificultad con el proceso de revisión de pares, abona el terreno de las revistas depredadoras, y las torna seductoras para los investigadores que no conocen su forma de operación. Por todo lo anterior, Gracias especiales a nuestros revisores pares que garantizan la calidad de nuestros procesos y artículos; a los miembros del Comité Científico y del Comité Editorial, por sus aportes significativos en la orientación del devenir de la revista para los próximos años; al personal de soporte técnico y profesional; a los investigadores que confían en la revista para someter sus artículos; al millón de lectores que anualmente nos visitan, y sobre todo, a la Sociedad Colombiana de Anestesiología y Reanimación (S.C.A.R.E.) que ha considerado éste como un proyecto bandera de la organización y por lo mismo, desde hace 45 años, financia su funcionamiento.</p>
